Asian & Pacific Islander Wellness Center's mission is to educate, support, empower, and advocate for Asian and Pacific Islander (A&PI) communities – particularly A&PIs living with, or at-risk for, HIV/AIDS. We are the oldest and largest nonprofit in North America focusing on sexual health and HIV in A&PI communities. Founded in 1987 as an all-volunteer, community-based response to the AIDS crisis in communities of color, we are based in San Francisco’s Tenderloin neighborhood with satellites in Alameda and San Mateo Counties, operate local, regional, statewide, and national programming, and collaborate with grassroots activists throughout the Asia Pacific Region. All of our work is aligned to fight HIV-related stigma, and focuses on A&PIs who experience the most extreme forms of discrimination and have the least access to quality services. Program Description:
HIV Care Services provides comprehensive integrated care services to over 200 HIV-positive Asians and Pacific Islanders annually. Its main office is in San Francisco’s Tenderloin area, with satellite offices in downtown Oakland and Daly City. The multi-disciplinary team is composed of multi-lingual and bicultural HIV Treatment Case Managers, HIV Peer Advocates, and a Clinical Program Manager/Psychotherapist along with onsite HIV primary care providers (physicians, nurse, health worker), a psychiatrist, an acupuncturist and massage therapist from external collaborating agencies. Peer leaders composed of consumers and volunteers provide valuable supplemental program support through outreach and direct client and program assistance.
Position Description:
The HIV Treatment Case Manager is responsible for (1) providing care and prevention case management; health and treatment education; service referral, linkage and coordination to Asians and Pacific Islanders living with HIV/AIDS and members of their family and support system (as needed); and (2) provide program support to HV testing and health screening services and capacity-building programs.
Major Responsibilities:
- Conduct intake, assessment and reassessment of eligible HIV-positive clients;
- Provide service referral, linkage, coordination and advocacy services for clients;
- Provide case management services following standards of practice;
- Develop and renew clients’ individualized case management treatment plans;
- Provide HIV treatment education and advocacy, as well as general health education;
- Provide prevention case management counseling and developing risk reduction plans with high-risk clients;
- Provide language support for risk reduction counseling and HIV testing for Asian & Pacific Islander at-risk target populations;
- Participate in and comply with agency quality assurance, peer review, and clinical consultation;
- Coordinate with onsite multi-disciplinary team to ensure appropriate level of services for clients;
- Coordinate with pertinent staff to ensure peer leader and volunteer supportive services for clients;
- Promote and coordinate program services, including representing agency/HIV Care Services program in community meetings as assigned;
- Assist in community events outreach as needed;
- Participate in agency and department events, activities, staff meetings, trainings, and other meetings as required; and
- Perform other duties as assigned.
Minimum Qualifications:
- BA or BS in a social science or health science related field ;
- 3-5 years counseling or case management experience with proven expertise in assessing and managing the psychosocial needs of persons with HIV/AIDS;
- Experience in working with multiply-diagnosed patients (HIV, mental health conditions and substance abuse), sexual and gender minorities, immigrants and refugees;
- Sensitivity to Asian and Pacific Islander cultural concerns as it relates to HIV;
- Ability to work independently as well as within a multidisciplinary team;
- Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills;
- Familiarity and experience with Microsoft Word and Outlook programs;
- Detail-oriented with strong follow-through skills, including attention to and respect for detail, accuracy, and deadlines;
- Must be able to handle multiple tasks and have ability to work professionally and ethically within multi-racial and multicultural settings; and
- Ability to maintain confidentiality and privacy of persons, documents, data, and communications.
Preferred Qualifications:
- Master's degree in social work, psychology or a related field from an accredited university and pre-licensure or licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners;
- At least three years experience working with HIV care in an acute setting and conducting individual and group psychotherapy strongly preferred;
- Written and verbal proficiency in an Asian or Pacific Islander language, specifically Thai or Lao;
- Sensitivity to HIV/AIDS and lesbian, gay, bisexual, and transgender issues; and
- Familiarity with data systems such as SPSS, SAS, or Reggie a plus.
