Crystal provides Prevention Case Management (PCM) to members
of the Asian & Pacific Islander Transgender community --
helping clients get the medical care and emotional support they
need to stay healthy and safe. As a Prevention Case Manager,
Crystal accompanies clients to appointments when needed and
can help with such issues as obtaining a legal name and gender
change, hormone therapy, drug rehabilitation and residential
treatment, employment training, SSI, medical care and other
services. |
